Formal. 'Intellectual' specifically refers to the ability to reason and understand, often implying high intelligence or academic pursuits. 'Mental' is broader, covering all aspects of the mind. 'An intellectual challenge' vs. 'A mental exercise'.

Formal/Legal (but also used informally). 'Insane' means 'mad' or 'mentally ill', often with legal implications regarding responsibility. It's less common and can be offensive in informal use, similar to 'mental' in this sense. 'Declared legally insane'.

Informal. 'Crazy' is a very common informal term for mentally unsound, but also widely used to describe something wild, exciting, or foolish without implying mental illness. When used for mental illness, it's considered impolite. 'That's a crazy idea!' vs. 'He went crazy'.
